Getting Started

To begin therapy with Seen, you'll need:

• A completed Therapy Intake Form (below)
  This completely confidential form is required for everyone. It helps us understand what’s been going on and whether we’re the right fit.

  If we feel another service may better meet your needs, a member of our team will be in touch to help guide you toward appropriate support.

• A GP referral (if using Medicare)
  If you’d like to claim a Medicare rebate, you’ll need a Mental Health Treatment Plan and referral from your GP. Instructions can be found here.

  OR if you’re attending as a private full-fee client, no referral is required — just your intake form.

  Once you’ve completed your intake form, you can proceed to book your appointment online.

  A 20% deposit is required at the time of booking to secure your appointment.
