This confidential form helps us understand what you are seeking, whether Seen is the right service for your needs, and what type of assessment or support may be appropriate.

Relevant documents

Please upload any helpful documents, such as previous reports, NDIS plans, school reports, GP letters, paediatrician letters, behaviour support plans, or previous assessment results.

Funding information

If you are using NDIS funding, we will need to know whether you are self-managed or plan-managed. Seen Psychology Services does not currently provide NDIA-managed services.

Submitting this form does not guarantee that Seen can provide the requested assessment or report. If we feel another service may better meet your needs, a member of our team will be in touch to help guide you toward appropriate support.
